    Focus on addressing the situation first rather that who is responsible for the situation. Once issue is resolved, then have a post implementation review with the sole objective of how we can improve further?, what were the missing link?,are our checks and balances strong enough? etc…Having the right framework and culture to deal with issues are important and should be part of organizational process and explained clearly to all employees.
